You want to ensure quality care for the lifetime of your child (or other relative), no matter what happens to you. Putting together the benefits, services, and care for your child is a nearly overwhelming responsibility. You need to know that all these pieces will be put together when you are no longer able to take care of them, and that everything will always be arranged according to your wishes.
 
With Coordinated Care of MetroWest (CCM), you have the opportunity to develop an individualized plan that will provide lifetime advocacy if you become incapacitated or wish to delegate responsibility, or upon your death. This plan will be maintained and carried out under CCM’s supervision.

Through a Community Trust (Special Needs) established at the Jewish Community Foundation, the enrolled family funds the unit(s) of social services needed for the special needs child.

A Powerful Partnership

CCM coordinates the benefits, services, and care that your child requires. To accomplish its mission, CCM works with two respected agencies: Jewish Family Service (JFS) of MetroWest and Jewish Community Foundation (JCF) of MetroWest NJ.

JFS handles the care management, which includes:

  • Designating a case manager who will maintain ongoing communication with your child.
  • Obtaining, coordinating, and monitoring services as they are needed from a variety of community agencies.
  • Serving as a liaison to trust officers or other financial planners who can also act on behalf of the beneficiary.
  • Maintaining contacts with the beneficiary’s extended environment, including financial, medical, residential, vocational, rehabilitative, educational, and social resources.
  •  Mediating among and advocating to existing structures and systems that directly influence the life of the beneficiary, i.e., Social Security, entitlements, residential needs, etc.
  • Providing counseling that is not covered by insurance and that supports the fulfillment of activities of daily living, self-care, judgment, and social behavior.

JCF manages the financial aspects of your plan; it holds and invests, according to your instructions, the money you have set aside for your child’s care. A variety of funding methods are available.

A contract with CCM funded by an appropriate financial plan will enroll your child in the Coordinated Care program. Let Coordinated Care of MetroWest complete the puzzle for your child when you no longer can.
